At the Ohio Center for Cosmetic Dentistry we are committed to providing for our patients the most caring and highest quality of service possible. We take our responsibility to our patients seriously and expect our patients to respect our office policies in return.
Method of Payment:
Unless specific prior arrangements have been made, full payment is due at the time of service. We will accept payment in cash, checks, Visa, MasterCard, American Express, or Discover Card.
For extensive treatment (over $300.00), we offer interest free payment plans for 3, 6 or 12 months. This payment plan is subject to prior credit approval. Pre-approval can be confirmed in seconds. If you wish to accept the pre-approval offer, a short application form can be completed at that time. After your care credit account is established, it can be debited for any treatment that is performed. Low interest rates are also available for periods of credit beyond 12 months.
For those patients who have dental coverage, we will estimate your benefits and the remaining portion will be due at the start of treatment. We will be more than happy to review your individual policy with you, as dental insurance terminology and applications can be quite complicated.
Please note that your insurance policy is a contract between you and your insurance company. Any fees not covered by your dental insurance are your responsibility. However, we will work with you to provide your insurance company with all of the necessary information to pay their rightful portion in a timely manner. The courtesy of completing and submitting your insurance forms to your carrier will be extended to all of our patients. We only ask that you keep us updated as to any changes in your insurance coverage.
For those patients who need multiple visits for extensive treatment, we offer discounts for payment in full at the first visit. 5% will be deducted from your out of pocket cost when the balance is paid in full by cash or check. 3% will be deducted when the balance is paid in full by credit card.
